Patient Care Coordinator

Altitude Physical Therapy is a therapist-owned clinic that opened its doors in 2005. Since then, we have grown and now have 3 clinics: one in Caldwell and two in Nampa, Idaho. We provide one-on-one physical therapy treatments in a warm, friendly and caring environment and we have 2 openings for a part-time Patient Care Coordinator.

Our Patient Care Coordinators are instrumental in helping patients receive great care with the attention and follow-up they deserve. This requires a flexible attitude, quick thinking and a caring disposition. Duties may vary from day-to-day and week-to-week, but will involve the following tasks and responsibilities:

  • Patient screening and scheduling
  • Communicate with practitioners
  • Handle inbound calls and answer patient questions
  • Connect with new patients via phone and email
  • Coordinate with insurance carriers
  • Increase patient visibility through community interaction, outreach events, and other activities
  • Maintain electronic files
  • Patient case management tasks
  • Help with data collection
  • Maintain patient confidentiality
  • Related duties as needed

The ideal candidate will have:

  • 1-2 years experience in a healthcare environment
  • Strong communication skills
  • Compassionate nature
  • Professional appearance and above average critical thinking skills
  • Experience with basic computer programs and scheduling software
  • Ability to work independently and be a self-starter
  • Customer service experience in a medical setting, preferred

Physical Therapist

Energetic Physical Therapist Needed in Physical Therapy Clinic (Nampa/Caldwell, ID)

We have a one-on-one treatment model where you have an hour each visit to spend quality time working with your patients to guarantee the best outcomes possible.

We are a dynamic, evidence-based physical therapy clinic specializing in orthopedic and sports medicine. We are growing and have an immediate opening for a physical therapist. Does this sound like you?

Our therapist needs to be a self-motivated, very organized individual with an advanced degree in physical therapy from an APTA-accredited physical therapy program. Must have a current (or have been eligible for) ID state license in good standing with the Idaho State Board of Physical Therapy. We are looking for full-time hours (35-40 hours a week), but are willing to be flexible for the right candidate. Options may include 4 10-hr workdays or other agreeable arrangements.

Duties and responsibilities will include:

  • Evaluating each patient (utilizing our 7-step exam) that is specific to age, illness, injury, and/or disease.
  • TIMELY preparing a written plan of care (using our web-based EMR) based on evaluation findings.
  • Documenting functional outcome tools as required by our payors.
  • Supervising the implementation of each plan of care, and coordinating treatment with support staff.
  • Tracking established clinic metrics for both therapist and administrative staff.
  • Actively participating in ongoing marketing strategies to ensure the health of our practice.
  • Willingly participating in community events to help promote our practice.
  • Being a proactive member of our team brings proposed solutions to any problems that may arise.
  • Helping foster a professional environment that is friendly and welcoming to everyone.
  • Displaying a willingness to go above and beyond to ensure each patient’s satisfaction.

Compensation for this salaried position will include a competitive hourly wage, performance-based incentives, health insurance stipend, PTO accrual, and continuing education allowance.
